All you need to give me is any required forms that they need me to fill out.

In addition, you must e-mail me the following (even if it is on the form):

-        The address that the recommendation needs to be sent to

-        The date the recommendation must be sent by

-        Whether you gave me a form to accompany the recommendation or if it is just a recommendation letter that is required

I will send you written confirmation soon after the recommendations are sent.
